Home Health Aide (HHA)

The home health aide provides personal care and related services in the home that are allowed to be performed under state law. She/he functions under the direction, instruction and supervision of the staff nurse and the Supervising Nurse and/or appropriate supervisor.


Qualifications:

  • Meets one of the following requirements:
    • Satisfactory completed a training & competency eval program that complies with the requirements of TX Code and CMS regulation (minimum 75 hours) and have a certificate of completion or
    • Satisfactory completed a competency eval program that complies with the TX Code and CMS regulation or
    • Successfully completed a nurse aid training/competency eval program approved by the state of TX as meeting the CMS and be listed on the TX DHS’s nurse aid registry in good standing, with no findings relating to abuse, neglect or misappropriation of client property or
    • Meets the requirements of a TX state licensure program that meets the provisions set forth by CMS Conditions of Participation for Home Health Agencies for Home Health Aide training & competency.
  • And has provided home health services within the previous 24 months
  • Prefer a high school graduate or equivalent
  • The necessary skills to perform their job responsibilities
  • Is registered with the TX Department of Human Services’ (DHS’s) NAR with no findings of client abuse or neglect or misappropriation of client’s property
  • The ability to read, write and communicate effectively in English, consistent with job requirements
  • Reliable means of transportation to get to assigned client visits
  • Must have a criminal background & other required checks and current CPR certification
Licensed Vocational Nurse (LVN)

A Licensed Vocational Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates clients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RN’s, Administrator, and Supervising Nurse.


Qualifications:

  • A graduate of an approved LVN nursing school
  • An active license to practice professional practical/vocational nursing in Texas
  • A minimum of one (1) year of experience as a LVN, preferably in a home care setting
  • Will have a criminal background check and any other required checks
  • Will have current CPR certification
Medical Social Worker (MSW)

The MSW provides medical social services within the scope of MSW practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates clients and their families. Medical Social Services in the home is performed.


Qualifications:

  • A Masters or Doctorate degree from a social work program accredited by the Council on Social Work Education
  • Will have a minimum of one (1) year experience in Social Work in a health care setting, preferably in the homecare setting
  • Currently licensed in TX
  • Will have a criminal background check and any other required checks
  • Must have current CPR certification
Social Worker Assistant (BSW)

The BSW provides medical social services within the scope of BSW practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates clients and their families. Licensed Social Services in the home is performed under the supervision of the Masters Medical Social Worker.


Qualifications:

  • A BA degree in Social Work, Psychology, sociology or a related field
  • A current license in TX
  • A minimum of 2 years’ experience providing social services in a health care setting, preferably home care providing social services
  • Will have a criminal background check and any other required checks and current CPR certification
Occupational Therapist (OT)

An OT provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates clients and their families. OT in the home is performed under the supervision of the RN case manager, Administrator, Supervising Nurse. If Nursing has opened the case, Occupational Therapy is a secondary discipline and therefore works collaboratively and professionally with the RN case manager to provide necessary services.


Qualifications:

  • Graduated after successful completion of an OT education Program accredited by the Accreditation Council for OT Education (ACOTE) of the American OT Association (AOTA) or successor organizations and is eligible to take or has successfully completed the entry level certification exam for OTs from the NBCOT
  • Currently licensed in TX
  • Will have a minimum of two (2) year experience as a OT, preferably in a homecare setting
  • Will have a criminal background check and any other required checks & Current CPR certification
  • If educated outside of the United States must meet both of the following:
    • Graduated after successful completion of an OT education program accredited as substantially equivalent to OT entry level education in the US by one of the following: Accreditation Council for OT Education (ACOTE), successor organizations of ACOTE, the World Federation of OTs, a credentialing body approved by the American OT Association, successfully completed the entry level certification exam for OTs developed & administered by the National Board for Certification in OCC Therapy (NBCOT)
    • On or before December 31, 2009, is licensed or otherwise regulated, if applicable, as an OT by the state in which practicing
Occupational Therapist Assistant (OTA)

Provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates clients and their families.


Qualifications:

  • A person who meets all of the following:
    • Is licensed or otherwise regulated, if applicable, as an OT assistant by the state in which practicing, unless licensure does apply
    • Graduated after successful completion of an OTA education program accredited by the Accreditation Council for Occupational Therapy Education, (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA) or its successor organizations
    • Is eligible to take or successfully completed the entry-level certification examination for OTAs developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
    • A minimum of two (2) years experience as an OTA, preferably in the homecare setting
    • Criminal background & other required checks & Current CPR certification
  • If educated outside the United States, on or after January 1, 2008—
    Graduated after successful completion of an occupational therapy assistant education program that is accredited as substantially equivalent to occupational therapist assistant entry level education in the United States by:
    • The Accreditation Council for Occupational Therapy Education (ACOTE)
    • Its successor organizations
    • The World Federation of Occupational Therapists
    • By a credentialing body approved by the American Occupational Therapy Association; and
    • Successfully completed the entry level certification examination for occupational therapy assistants developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
Personal Care Worker (PCW)

A person, who under professional supervision, provides assistance with nutritional and environmental support, personal hygiene, feeding and dressing.


Qualifications:

  • Successful completion of a state approved program or a 40 hour training equivalent in PCA skills
  • Shows an interest and concern for people, good judgment and tact in dealing with the sick
  • Practices good hygiene and is neat in personal appearance
  • Speaks, reads, and writes English
  • Must have a criminal background check, other required checks, and current CPR certification
Physical Therapist (PT)

A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates clients and their families.


Qualifications:

  • Successful completion & graduate of a PT Program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists
  • A current, active license to practice Physical Therapy in TX
  • A minimum of two (2) years’ experience as a Physical Therapist
  • Will have a criminal background check and any other required check & Current CPR certification
  • If trained outside of the United States before January 1, 2008, meets the following requirements:
    • Was graduated since 1928 from a PT curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Conference for Physical Therapy
    • Meets the requirements for membership in a membership organization of the World Confederation for Physical Therapy
Physical Therapist Assistant (PTA)

Under the supervision of the Physical Therapist and according to established treatment plans and specific procedures, provides treatment to physical therapy clients in order to assist them in regaining physical mobility and function, increase strength, and decrease pain.


Qualifications:

  • A person who is licensed, registered or certified as a PTA, if applicable, by the state in which practicing, unless licensure does not apply, and meets the following requirements:
    • Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association
    • Current PTA license in the state of practice
    • A minimum of two (2) years experience as a PTA. preferably in home health care
    • Criminal background check & and other state required checks for compliance
    • Current CPR certification
  • If educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to PTA entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e); and
  • Passed a national examination for physical therapist assistants
  • Has a current PTA license in the state of practice
  • Has a minimum of two (2) years experience as a PTA, preferably in the homecare setting
Registered Nurse (RN)

A professional who provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates clients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Supervising Nurse.


Qualifications:

  • An active license to practice professional nursing in Texas
  • Preferably completed a BA degree program
  • A minimum two (2) years of experience as a professional nurse, with at least (1) year of experience, preferably in community/home healthcare
  • Will have a criminal background check and any other required checks
  • Must have current CPR certification
Speech-Language Pathologist (SLP)

A Speech Pathologist/Therapist provides professional services within the scope of Speech Pathologist practice standards in collaboration with primary care physicians, teaches and educates clients and their families, and evaluates and assesses client’s therapy needs.


Qualifications:

  • A person who has a masters or doctoral degree in speech-language pathology and who meets either of the following requirements:
    • Licensed as a speech-language pathologist in the state in which the individual furnishes services
    • OR In the case that the individual provides services in a state that does not license speech-language pathologists, has successfully completed 350 clock hours of supervised clinical practicum (or in the process of accumulating supervised clinical experience) AND performed not less than 9 months of supervised full-time speech-language pathology services after obtaining a masters or doctoral degree in speech-language pathology or a related field AND has successfully completed a national examination in speech-language pathology approved by the secretary
  • Will have a criminal background check and any other required checks
  • Must have current CPR certification
