The Agency’s mission is to provide quality, reliable services to you. Our professional staff delivers the highest standards in home care nursing, therapies, and medical social work. Our Paraprofessional staff work tirelessly to assist you with those activities of daily living that are so critical to your independence and ability to stay in your own home as long as possible. Our administrative and office staffs coordinate all services to provide seamless, effortless service for you. There is always someone for you to call when you have changes or need questions answered.
Our Approach
At Gold Standard Home Health LLC, we collaborate with your physicians and case managers (in some cases), hospitals and rehabilitation facilities, to provide the services you need when you need them. Our Agency maintains a medical record of the services we provide to you including documentation of each discipline’s visits, MD orders, progress notes/communication notes and OASIS assessments. Your record is secured, and its privacy protected at all times. You may request a copy of your record by sending your request to us in writing. By signing your admission documents, you are authorizing our Agency to collect and maintain that record by either paper charts or electronic medical record.
Always Available
You can contact us Mon-Fri 9-5 P.M. at our office phone. After normal business hours, should you need assistance you can call us through our answering service by calling our regular phone number which is forwarded to on call after hours. Either our on-call nurse or on call scheduler will return your call.
